Presentation from: Nutrition Services in Primary Care

A showcase of the different nutrition services offered in primary care and communities.

The purpose is to help people with chronic conditions like diabetes and CVD and to help people live a healthier life to avoid these conditions.

Everyone is welcome and light refreshments will be provided
